To investigate the effect of atrial fibrillation on the incidence of intracranial hemorrhage (ICH) in patients with acute ischemic stroke (AIS) treated with tirofiban. The present study involved 2,306 consecutive patients with AIS who underwent tirofiban treatment. The follow-up lasted for 7 days. The incidence of ICH was analyzed for patients with and without atrial fibrillation (AF). The prognostic value of ICH was assessed for the patients with AF. In total, 602 patients (25.2%) were diagnosed with AF. A total of 105 (6.9%) patients developed ICH after tirofiban treatment. AF was present in 93 patients (6.3%) and ICH occurred in 15 (16.1%) of these patients. A logistic regression analysis showed that AF was an independent risk factor for ICH (OR = 1.57, 95% CI = 1.16-2.12, p = 0.003). The multivariate logistic regression analysis showed that AF was an independent risk factor for ICH (OR = 2.52, 95% CI = 1.40-4.55, p = 0.002). In the 1,230 patients without AF, 85 (6.9%) patients developed ICH after tirofiban treatment. In the 1,275 patients without AF, ICH occurred in 94 (7.4%) patients. No significant difference was found between the two groups (p = 0.09). AF is an independent risk factor for ICH after tirofiban treatment. AF is more likely to occur in patients with ICH. Therefore, it is suggested that AF be treated in the acute phase of ischemic stroke.Q:
